Don't lose biometric card

In this post I will guide you how to apply for a new biometric card, if you lost it or got stolen (wallet etc.)

First of all, be sure that you really lost it and cannot find it, cause next you will see how frustrating to restore it;

Immediately go to nearest police station; let's say you noticed loss at home, then go to police station of your address' municipality (part of city). Officers mostly speak in English, if not then your show them following text:

Dobrý den, 

Dovolte mi oznámit krádež peněženky se všemi mými doklady, včetně povolení k dlouhodobému pobytu-zaměstnanecké karty. Prosím o vystavení potvrzení o nahlášení krádeže, abych mohl na Odboru azylové a migrační politiky zažádat o vystavení nové pobytové karty. Krádež jsem na OAMP ze zákona povinnen nahlásit do 3 pracovních ní.

Basically, it says that you want to report loss or stolen wallet, so you can get a report paper. This report is needed when you will also report in Immigration Office (OAMP) and apply for a new one. By the law you obligated to do in 3 working days.

After having report from Police you have to go to OAMP and take queue in Long Term Residency. Yes, as we all know these days queue is LONG and it takes hours and hours. But this is "price" of losing card. I do recommend to go there at 6 or even 5AM, no kidding. 

While you will wait for your "number to pop up", fill the "green application form": your last name(s), first name(s), date of birth, gender and in the line 15 write "ZTRÁTA KARTY", which means lost of card.  At the last page place, date and signature needed. 

If you need/want confirmation paper, which will proof registered address in Czech Republic, while you will be without biometric card, you can ask for it. It costs 15 CZK, so buy stamps in advance.

They will provide you with paper; Hlášní - Potvrzení o ohlášení ztráty dokladu. This paper VERY important and you will need show in order get new card. If you don't show it immediately, usually they expect you to return old one, they will get confused and hard to explain. 

After OAMP employee finishes your request you will be forward to biometric photo taking room. They will get photo and fingerprints as usual and tell date/time to come and receive new biometric card.

And here is another downside of loosing card; when you will receive new card, have to pay 4000 CZK (as stamps), this is fine for loosing or damaging card.
