In this post I will guide you how to apply for a new driving license in case your lost it or got stolen (wallet etc.)
Note that you don't need police report. You will have to submit following documents in person at the Drivers Agenda Section of Brno City Municipality, Kounicova 67, Brno:
Till license will be ready to pickup within 20 days, they will provide temporary paper driving license. Cost of restore is 200 CZK.
It is better to have biometric card first (in case of you lost it too), then go and pickup new driving license.

All in all, try not to loose driving license (too).

- First of all, you will have to identify yourself with biometric card (long-term stay permit etc.). In case your lost biometric card too, you will have to provide paper which proofs accommodation address and type of permit (long-term stay etc.). You can have this paper from Immigration office, see Don't lose biometric card post.
- Proof of accommodation; in case of rent, rental contract. Contract should not be older that 180 days, so you can have new one or ask supplement (dodatek in Czech) form landlord.
- If you read my first post How I got my Driving Licenses (DL) in Brno, you can see that I had to come with my wife and proof that we live together in same address. I know weird. So, they will need (at least in my case) affidavit (čestné prohlášení); either spouse has to come and sign it in front of them or sign in any Post office (CzechPoint), which they can stamp with verification (notarize basically). I could not find exactly same example on internet, so I will update post later on. For now you will have to take blank page from them.
- And as always have your passport with yourself.
